Termination Form
A Termination Form records an employee's exit - the Reason for Termination (a master list covering things like Voluntary Separation, Dismissal for Misconduct, Employee Deceased, Transfer of Employment, or End of Fixed-Term Contract), a Termination Date, and an ID number for the record.
This is distinct from a Dismissal Form (see Issuing a Sanction) - a Dismissal Form is specifically the sanction outcome of a disciplinary case; a Termination Form covers every other way an employment relationship ends, including entirely voluntary ones.
